You must enter your legal name in the "Name" field in order to connect it to the dance name you are claiming. The only name that will show in the Registry is your dance name, along with your area of establishment and style/tradition. Any duplication of names will be notified by e-mail of the existance of usage elsewhere (it is a personal decision of the dancers involved how to resolve this; ASAmed does not bear responsibility or will in any way arbitrate). Our only purpose is to have a central registration of dancers' names in use by whom and where.
Please note that it may take several days before your submission appears in the Registry, as the information submitted must be listed manually on the registry pages.

Once you have submitted the form you will get a page with a button that says"continue." Clicking "continue" again will only get you another form like the one you just completed as a new form automatically appears for additional name registrations.
